I hereby consent to authorize Bird Communications (the producers of Ready for My Shot) the unrestricted right to use and publish written products, photographs, audio and videos of me or of my child, sibling or ward, for editorial, promotional, and any other reasonable purpose related to the Ready for My Shot campaign.

I understand that these images and words will be shared on the Internet and seen by the public.

I hereby release Bird Communications and its contractors from all claims and liabilities relating to said products.   

   

By checking the consent box on this website, I understand and agree that all communications materials will remain the property of Bird Communications unless consent is withdrawn.

I understand that I can withdraw this consent at a future time, however content that has previously been released in any form (including posted on the Internet) cannot be retrieved. If consent is withdrawn, Bird Communications will delete the material from this website.
